No one plans to get injured or need assistance, but it is a welcome relief to know that if you need us, the region’s premier Transitional Care Units (TCU) are here for you when it matters most. Your physician has recommended transitional care to help your recovery process progress as safely and quickly as possible.

Our transitional care services are available on a short-term basis for those who are well enough to leave hospital care after an injury, illness or surgery but not quite ready to resume all of the daily activities at home without assistance.

Most TCU patients require only 7-10 days of assistance before they are able to return home.

The teams at each TCU facility are dedicated to helping you heal, restore strength and re-establish your independence.
Your team of physicians, therapists, nurses and caregivers will meet weekly to evaluate your progress and develop your personal care plan.They will also help you and your family learn what you need to know about your continued health upon discharge. If you qualify, the fees associated with short term TCU stays are covered by Medicare and some private insurance companies, provided you received acute care for a minimum of three days in a hospital. Your physician must also identify the need for additional skilled care such as daily IV medication, tube feedings, wound care, physical therapy, occupational therapy or speech therapy.
